Surgical and Mechanical Alterations of the Alveolar Bone to Facilitate Tooth Movement; The Biologic Basis and Historical Perspective
There has been an increase in the amount of clinical research and commercial activity focusing on accelerating methods for tooth movement. While surgical and mechanical interventions affecting the alveolar housing and tooth movement has been described for over a hundred years, the demand to find the best method to increase tooth movement with the least possible disadvantages continues. The purpose of this presentation is to outline the clinical techniques, discuss the historical perspectives and provide the proposed biologic basis for these techniques. Techniques described will include the mechanical, surgical, low-level laser and resonance vibration alterations of the alveolar bone. An analysis of available long-term clinical data will also be presented.
At the Conclusion of this lecture participants will have gained an understanding of:
1. The historical background of surgically facilitated orthodontics
2. The possible biological mechanisms for enhanced tooth movement after alveolar bone alteration

Surgically Facilitated Orthodontic Therapy (SFOT): The “Clear” Direction
Many complex interdisciplinary problems involve deficiencies or discrepancies in the dentoalveolar complex that can prolong treatment time and compromise overall outcomes. Inter-arch and intra-arch space appropriation problems are most evident as poor esthetics or less than ideal occlusions. Inter-arch problems are caused by TM joint changes or growth and development issues. Intra-arc problems manifeset themselves as poor arch forms, severely crowded dentitions, insufficient alveolar bone and traumatic or developmental ridge defects. Surgically facilitated orthodontic therapy (SFOT) in combination with skeletal anchorage can shorten treatment times, modify vertical relationships and enhance the development of the arch form, alveolar bone and soft tissue to ideally address these issues involving the dentoalveolar complex. Special focus will be given to utilizing high-tech clear aligners instead of traditional orthodontic appliances to add a new and exciting dimension to SFOT while significantly increasing patient treatment acceptance.
At the conclusion of this lecture participants will have gained an understanding of:
1. Interdisciplinary diagnostic & treatment planning the SFOT patient
2. Combining hard & soft tissue grafting with selective corticotomics
3. Utilizing skeletal anchorage with SFOT to modify vertical relationships
4. High tech clear aligners as an exciting new appliance option for SFOT

Dentistry that is esthetic to the patient is an important clinical objective. The knowledge within dental technology, dental science and dental practice has dramatically expanded leading to better quality; artistry and more standards based clinical applications. Ceramics are the most consistently predictable esthetic dental material. Today dentists can offer more treatment options for patient’s complex problems. Single phase or monolithic all-ceramic materials have become increasingly more popular and have become the “state of the art” in single unit complete or partial restorations. In clinical use they have not demonstrated the chipping problem that often can occur with all bi-layered ceramic materials. These types of dental ceramic materials will dominate the market and future developments bring with it more long-term success. Metal-ceramics continue to the most predictable and esthetic for complex prosthodontic care
Understanding methods to manage simple and complex restorative issues are critical to improving patient acceptance and even long term ceramic success.
